This Privacy Notice applies to the UHC Patient Portal, which is owned and operated by Union Health Center (“UHC”).  Any use of this Web site constitutes your agreement with this Privacy Notice.  Please be advised that the practices described in this Privacy Notice apply only to information gathered through and in connection with this Web site and do not necessarily reflect our practices elsewhere.

 

Personal Information Collection

 

In connection with your use of UHC services and this Web site, UHC may collect or receive certain personally identifiable information (“PII”) and information that is “Protected Health Information” (“PHI”) under the Privacy Rule issued pursuant to the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).  PII is defined as information in any form (including electronic form) that can be traced back to an individual, including date of birth, first and last name, and member record number.  PHI is defined as information in electronic or other form that relates to the past, present, or future physical or mental health condition of an individual, the provision of health care to an individual, or the past, present, or future payment for the provision of health care to an individual if such information identifies the individual or with respect to which there is a reasonable basis the information can be used to identify the individual.

 

Information Collected by Your Use of the Web site

 

You may enter PII or PHI voluntarily when you visit this Web site, access various health-related topics and tools, and submit your information, as applicable.  UHC may also collect certain personal and non-personal information related to your use of the Web site.  For example, we may track your domain name, IP address, and browser type. 

 

Use of Personal Information

 

UHC may use or disclose your PII and PHI as follows: (i) in order to provide services to you, (ii) for treatment, payment, and health care operations, including the proper administration and management of our business and this Web site, and (iii) to fulfill any legal responsibilities and as permitted by law.  UHC will not use or disclose your PII or PHI to any third party without your authorization, except as set forth in this Privacy Notice or as required by law. 

 

In order to provide services and to satisfy our responsibilities as set forth above, UHC may disclose your PII and PHI to our employees and affiliates, who need access to the PII or PHI to perform their duties and who are obligated to maintain its confidentiality.  In such cases, we will provide the minimum PII and PHI necessary to accomplish the purpose for which it is used or disclosed.  We may also disclose PII or PHI to certain agents, service providers, or other third parties of UHC for the purposes set forth above.  If we disclose your PII or PHI to any such third parties (other than government officials or agencies), we will require that the recipients of such information agree to maintain its confidentiality.

 

You also acknowledge and agree that we may disclose PII or PHI in our possession in connection with a business transaction such as merger, acquisition, or any form of sale of all or a portion of assets or in the event that an emergency situation is deemed to occur or is likely to occur and use of the information could help prevent or mitigate harm.  

 

Aggregate Data Collection


UHC may combine, PII and PHI in our possession with information from other users to create aggregate data that reflects the habits and characteristics of an anonymous group (“Aggregate Data”), which can be used to show general trends among the group.  This Aggregate Data can be used to improve and enhance the Web site to make it more useful to users.  UHC may provide Aggregate Data to third parties.  We require third parties with whom we share Aggregate Data to agree that they will not attempt to make the information personally identifiable. 

 

Security and Integrity

 

We employ safeguards to protect the confidentiality, integrity, and availability of your PII and PHI.  PII and PHI is stored in a secured database designed to protect its privacy and security.  We take all reasonable steps to prevent unauthorized use or disclosure of PII and PHI.  However, despite UHC’s efforts to protect your information, we cannot guarantee and do not warrant or represent, that all information will be protected against loss, misuse, or unauthorized access by third parties.

Upon receipt of a request you, we will delete your PII or PHI from our active databases and, where feasible, from our back-up media. Please be aware that it is not technologically possible to remove each and every data record from our servers.

 

Modifications to Privacy Notice

 

Any modifications made to this Privacy Notice will be effective immediately upon posting on the Web site.  When the Privacy Notice is updated, UHC will post a new effective date below.  You agree to review the Privacy Notice posted on the Web site each time you use the Web site so that you are aware of any modifications made to the Privacy Notice.
